Warranty Procedures
- Download and print the Warranty Claim Form.
- Fill out claim form.
- Fax completed form to 574-753-6261 or email to warranty@bullseyeleveling.com.
- Obtain an authorization number.
- Bullseye will ship necessary parts, freight prepaid.
- Parts to be returned to Bullseye upon request.
- Complete repair work.
- Submit completed form for labor reimbursement.
